Often, not having access to a primary care provider results in a hospital presentation for aged
care residents. Our doctors visit aged care facilities (ACF) after-hours during the week; on
weekends and on public holidays. All Medicare and DVA card holders are bulk-billed. In addition
to providing the necessary medical care, the resident’s regular GP is securely sent an
electronic copy of the consultation report within 24 hours of the consultation to ensure
record-keeping and follow-up.

Assessment and treatment of confusion and BPSD in patients with dementia.
Simple wound care management including dressing review and assessment of need for suturing or gluing.
Prescription of S8 medications where appropriate and clinically indicated.
Ongoing prescription renewal as part of medication management plan by regular practitioner and pharmacy.
Ongoing orders for symptomatic palliative care and end of life care provided in accordance with Advanced care Plans.
